Why measure C3NeF?

C3 nephritic factor (C3NeF) is an autoantibody that binds to and stabilizes the alternative pathway C3 convertase (C3bBb). By preventing normal breakdown of this enzyme complex, C3NeF promotes continuous activation of the complement system.

Persistent complement activation may result in increased C3 consumption, chronically low circulating C3 levels, and ongoing inflammatory activity. As a result, C3NeF has become an important biomarker in research on complement-mediated diseases.

Clinical relevance in rare kidney diseases

C3NeF is frequently associated with rare complement-mediated renal disorders, including:

  • C3 glomerulopathy (C3G)
  • Membranoproliferative glomerulonephritis (MPGN)
  • Atypical hemolytic uremic syndrome (aHUS)

These disorders are characterized by abnormal activation of the alternative complement pathway and deposition of complement components in the glomeruli, which can lead to progressive kidney damage.

Many affected patients develop symptoms during childhood, adolescence, or early adulthood. Clinical manifestations may include:

  • Proteinuria
  • Hematuria
  • Edema
  • Reduced kidney function

Because early symptoms can be subtle and disease progression is highly variable, improved understanding of complement dysregulation remains an important focus of current research.

Supporting complement research and therapeutic development

Growing insight into the role of complement activation in kidney disease has led to increased interest in targeted therapies aimed at the alternative pathway.

Several ongoing clinical studies are investigating treatments that inhibit complement components such as C3 or Factor B. Reliable detection of C3NeF may help researchers better characterize patient populations and study mechanisms underlying complement-mediated disease.

The C3NeF detection ELISA kit (Cat.#HK3051) enables in vitro qualitative detection of C3NeF autoantibodies in heat-inactivated serum or plasma samples.

Assay principle

The assay is based on a C3b-coated microplate. Functional Factor B (FB) and Factor D (FD) are added to generate the C3bBb convertase complex. When present, C3NeF antibodies bind to this complex and are subsequently detected using an HRP-conjugated anti-human IgG antibody. Signal generation is achieved through enzymatic conversion of the TMB substrate.

As a prescreening tool, the assay can support identification of samples with potential alternative pathway dysregulation.

Because the presence of C3NeF antibodies does not necessarily indicate functional complement activation, follow-up assessment using functional complement assays is recommended for comprehensive characterization.
